Standard of care analysis

A written clinical opinion on whether care met the accepted standard, cited to the record and to the sources it rests on.

The question is never whether the outcome was bad. It is whether what the clinicians did fell outside what a reasonable clinician in that role, in that setting, with that information, would have done. Answering it takes someone who has actually held the role.

The analysis names the standard, names the source, applies it to the documented facts, and states plainly where care met it and where it did not — including the places your theory does not survive contact with the record.
