The record and the case theory should say the same thing.
Most medical records come back as notes, orders, and results. That is not the same as understanding what happened. Jose Rangel currently manages a large and busy Emergency Department and has served as an ER nurse, flight nurse, critical care transport nurse, nursing clinical instructor, and Trauma Team Lead at a Level II center. He reads a chart the way the people who wrote it were thinking when they wrote it — including what they left out.
