Medical record review

A complete read of every page in the production, by a clinician who has worked in the units that generated it.

Records arrive incomplete, out of order, and full of contradictions between what the flowsheet says and what the narrative note says. A reviewer without bedside experience reconciles those by picking one. A clinician knows which one is more likely to be true, and why.

The review covers hospital charts, EMS run sheets, provider notes, imaging reports, medication administration records, and nursing documentation. Findings come back as an annotated memo keyed to Bates numbers, organized by issue rather than by document, so you can go straight to the page that matters.
